Our Level of Service

This level of service is available for an annual fee (payable monthly) for medical services that are not covered by one’s health insurance including but not limited to 24/7 physician availability and unlimited telemedicine appointments. Requires 30 Day cancellation notice.

Chastain Pediatrics does not take insurance for physician related fees. Fees such as immunizations and lab testing can be submitted to your insurance company for reimbursement. Please check with your insurance company to see if these you can use your FSA / HSA account funds for these services. Please also contact your insurer to see if they will reimburse you for well child visits.  Chastain Pediatrics will provide well and sick medical services as requested by you, including but not limited to well checkups and immunizations. An itemized statement can be provided for your submission to the insurance company. 

Chastain Pediatrics does offer a fee-for-service rate for non-members and one-time visitors. Although, we strongly encourage families to join as a member to reap the many benefits of ongoing personalized care.
